Consent to Allow Your Athlete To Participate

Confidentiality: Data is stored securely, and only authorized personnel, such as the program administrator, will have access to it. No personal information will be shared outside the program.

Voluntary Participation: While coaches may encourage all team members to register and complete assessments, participation on the Assessdo Assessment Portal is completely voluntary without penalty or negative consequences. Coaches only use the Assessdo AI to build on their teams and can usually only do so with all team members participating.

Consent Form: A consent form is necessary before conducting assessments on athletes under 18 to ensure legal protection and respect for the athlete’s privacy rights. Minors (those under 18) cannot legally provide consent on their own. The form ensures that a parent or guardian gives permission for their child to participate in assessments, meeting legal and ethical standards. The consent form informs parents and athletes about what data will be collected, how it will be used, and who will have access to it, protecting the athlete’s personal and sensitive information.  It ensures that both the athlete and their guardians understand the nature of the assessment, the purpose behind it, and any potential risks, allowing them to make an informed decision about participation. Consent upholds ethical principles, ensuring that coaches, organizations, and technology providers respect the autonomy and well-being of the young athletes. Providing clear information about assessments builds trust between coaches, parents, and athletes, ensuring everyone is on the same page about the purpose and process of the evaluation. This form ensures that the assessments are conducted legally, ethically, and with the full understanding and agreement of both the athlete and their guardian.
